Abstract
This study summarized the nursing experience of integrated traditional Chinese and Western medicine based on the enhanced recovery after surgery (ERAS) concept in one patient with acute severe perianal necrotizing fasciitis after operation. The critically ill patient suffered from postoperative pain, anxiety, malnutrition and gastrointestinal dysfunction, resulting in great nursing difficulties. Guided by ERAS perioperative management and Traditional Chinese Medicine syndrome differentiation, routine Western nursing and standardized wound dressing were performed. Meanwhile, comprehensive TCM interventions including herbal warm fomentation, auricular point pressing with beans and acupoint application were adopted, combined with hospital-made Chinese herbal sitz bath. The integrative nursing interventions effectively alleviated pain and anxiety, improved gastrointestinal function and nutritional status, accelerated granulation tissue growth and wound repair, shortened hospital stay, reduced risk of complications, and improved patient comfort and satisfaction. The nursing practice may provide a feasible reference for postoperative nursing of severe perianal necrotizing fasciitis. (本文总结1例急性重症肛周坏死性筋膜炎术后患者的中西医结合护理经验。该患者病情危重, 伴焦虑、营养不良、疼痛等多重护理问题。护理人员经充分评估, 以加速康复外科围手术期管理为核心, 融合中医整体观念与辨证施护, 在西医常规护理及创面专科换药基础上, 联合应用中药热奄包、耳穴埋豆及穴位贴敷等中医外治技术, 配合院内中药制剂坐浴外治创面。中西医协同护理可显著缓解患者焦虑情绪, 减轻术后疼痛, 改善胃肠功能, 纠正营养失衡, 促进肉芽组织生长及创面修复, 缩短住院时间, 减少并发症发生, 提升康复体验与满意度, 可为同类重症肛周感染术后护理提供临床参考。)
