Designing Continuity Beyond Organisational Boundaries: A lightweight service design approach to improving care transitions

At 10:30 p.m., a 72-year-old patient from rural Northern California who we call Joe, was discharged from hospital after emergency surgery with no transportation home and nowhere to stay. He spent the night sleeping rough. Days later, he was readmitted with complications that ultimately rendered the surgery unsuccessful.
